Fresno

Crestwood's Fresno Bridge Program is a social rehabilitation facility, licensed by Community Care Licensing (CCL), a Department of Mental Health that certifies Transitional Residential Treatment programs. Persons served participating in our Bridge Program are preparing themselves to transition back into their communities and live independently. In partnership with staff, the people we serve develop and implement an individualized recovery-based plan that focuses on wellness in personal and community living. Our program also strongly believes in family support and encourages family participation during the recovery process. Our family workshops have been very successful in helping the people we serve develop a support system within their immediate family.

At Fresno Bridge, we focus on the foundational principle that wellness and recovery are an ongoing process that require active participation from persons served. Since the program's inception, persons served at Fresno Bridge have made great strides in their recovery. Our unique recovery model is based on Crestwood’s Four Pillars of Recovery: Hope, Empowerment, Meaningful Roles, and Spirituality. As we implement innovative recovery treatment plans centered on our values, we are able to empower persons served to reclaim control of their lives. By providing meaningful support to persons served, we are able to help them take progressive steps towards assimilating into the community, whether it is by continuing their education, developing skills for independent living, learning about volunteer opportunities, or furthering their skills for potential job opportunities.

The Fresno Bridge program is a licensed Adult Residential Facility and a California Department of Health Care Services certified Social Rehabilitation program. The Fresno Bridge program focuses on providing persons served with life skills training and residential rehabilitation treatment that helps them take on an active role in reaching their recovery potential. In serving these individuals, we prepare persons served to successfully transition back into their communities. As the people we serve partner with staff, they develop and implement an individualized recovery-based plan that focus on creating transformational habits that promote personal empowerment and stability. Through Crestwood's Fresno Bridge program, our goal is to help persons served take the necessary steps towards successfully reentering the community.
