Fremont Manor
Crestwood Manor Fremont is a 126-bed Skilled Nursing Facility that provides 24-hour nursing services for individuals with medical and behavioral diagnoses. The campus has two units. The larger unit is a Skilled Nursing Facility program with 74 beds, and it is designed for residents with a primary medical diagnosis and/or dementia diagnosis, as well as behaviors that may prevent placement in a lower-level of care. The smaller unit has 52 beds and is a Special Treatment Program that is designed for individuals with a psychiatric diagnosis and medical condition.
Crestwood Manor offers individual care plans and an array of groups and activities for our residents, including Zumba, Wellness Recovery Action Plan (WRAP) and art classes. We also provide full-service laundry, housekeeping, therapeutic dietary services, in-house podiatry, dental, and eye care. At Crestwood Manor Fremont, we aim to create a homelike environment for our residents, along with pet therapy, a large organic garden, and serenity rooms furnished with plush couches, books, and a television.

Crestwood Manor
At Fremont Manor, our 74-bed Skilled Nursing Facility (SNF) is designed to care for individuals with a primary medical diagnosis, dementia, or behaviors that may prevent placement in a lower level of care. Our persons served are typically individuals with long-term stay and are under conservatorship. The focus of this program is to provide persons served with the highest quality of life and encourage them to attain their highest level of functioning. Our SNF offers an assortment of life skill and recovery groups for persons served, including personal care, exercise, banking, and aroma therapy. Because we are not Medicare certified, each person that we serve is considered individually to ensure we can provide the proper level of medical care needed.
We also provide a 52-bed Special Treatment Program (STP) that is designed to provide the skills necessary for persons served to improve their current level of functioning. Through our STP, we can assist persons served in discharging to a lower level of care by offering a range of services, including trauma-informed care groups led by a licensed therapist and individualized care plans. Persons served also receive a primary psychiatric diagnosis and are supported by our staff while under LPS conservatorship.
